Part IV Maintenance of Certification (MOC) Support
For Clinicians and Practices
The Patient Experience Module is an assessment of patient experience including patient enablement, access, comprehensiveness, coordination, relationship, and suggestions on how to improve the practice. In meeting with the local family physicians, patient experience was identified as a key area of concern and opportunity. With increasing emphasis on the patient centered approaches to primary care, quality improvement projects, such as the Patient Experience Module, aimed at improving patient experience would allow family physicians completing their Part 4 MOC requirements to advance the fundamental tenets of primary care articulated by the IOM.
Brief Process Outline:
*Activities will be tracked by the CWRU facilitator and upon completion, a report will be sent to ABFM verifying completion of the requirements for MOC Part IV credit by each participating clinician.

Brief Process Outline:
- A CWRU practice facilitator will visit the practice and provide a general overview to participating clinicians. Instructions for distributing and collecting surveys will be provided to staff.
- A 3-5 day window will be selected for baseline surveys. Each patient seen during that window will be asked to complete a small portion of the survey prior to the visit and a small portion after the visit. The survey should take no more than 5 minutes to complete. Patients will be asked to return the surveys before leaving the office. Completed surveys will be placed in a collection box.
- A practice facilitator will pick up the completed surveys and process the results. A meeting will be scheduled for 10-14 days after surveys have been received by the facilitator. All participating physicians and the practice team will review the results and plan an improvement intervention. This process will be facilitated by the CWRU practice facilitator.
- The intervention will be implemented for a minimum of 1-3 months.
- A follow up survey will be done using the same procedures as the baseline survey. A final meeting will be scheduled to discuss the results and assess the overall activity.
